Important drugs

 Anaesthesiologists should always remember few facts about few important drugs without fail. Firstly, the drug of choice in surviving sepsis campaign is definitely Norepinephrine. Dobutamine is the drug of choice in post operative cardiac surgery cases. Renal dose of dopamine is a myth because it doesn't increase GFR and it's better avoided for fear of arrythmias. Phenylephrine is pure alpha 1 agonist and it lasts only for fifteen minutes. Avoid giving more than 2 microgram per kg per minute for fear of severe vasoconstriction and ischemia. Ephedrine lasts for one hour. But it's a tendency of causing foetal acidosis by decreasing uteroplacental perfusion due to its vasoconstriction. Ephedrine releases Norepinephrine from nerve terminals and produces its effects. When hypotension occurs after general anaesthesia, volatile  extra usage or after spinal anaesthesia, ephedrine is the drug of choice. But it can cause tachyphylaxis by its prolonged usage as the Norepinephrine stores get depleted. Among the two...amrinone and milrinone, milrinone is preferred as it doesn't cause thrombocytopenia. Milrinone increases the cyclic AMP by inhibiting phosphodiastrase and thus calcium entry increased and myocardial contractility too. When there's refractive hypotension in septic shock patients, try Vasopressin but it can cause diabetes insipidus and hyponatremia... Col Pradeep Pendyala

Humour and intelligence

 I always believed that people who've good sense humour are generally more intelligent. The ability to produce humour is a sign of cleverness. Funny people are always loved in the society and they do have higher IQ  ( intelligence quotient). Moreover, higher humour sense is linked to emotional intelligence. Comedians and mimicry artists usually have very good sense of observation. More curiosity means more intelligence. Someone actually said that you can tell how smart people are by what they laugh at. Western movies

 As a child, I saw many western movies ... The good, the bad and the ugly was definitely my favourite. But those days, there were no subtitles and the dialogues were very hard to understand.  Everyone was seen killing each other... It's actually very difficult to know who's the good guy and who's the villain. All were good looking guys unlike Bollywood movies... You can easily make out who's the Hero. Many years later, once my nephew Akshit asked me, uncle, who's the bad guy in the movie... My answer was... I'm trying to figure out myself ๐Ÿ™‚Col Pradeep Pendyala

Nostalgia

 We all miss our childhood but more so, people of my contemporary age. I can never forget the lovely Circus I saw as a child. I miss my picnics and the fairs which we attended so very often. I miss my simple and humble teachers teaching on the blackboard. I definitely miss the magical feeling of Sankranti and Durga Puja. Field Marshal Kodandera Madappa Cariappa

 Field Marshal Kodandera Madappa Cariappa sir is known to me. I'm sure, you must be surprised by this revelation. I was posted at Command Hospital Air force Bangalore between 1989- 1992 and I used to work regularly as a Duty medical officer... almost once every month and one of the charter of my duties as a DMO was to meet the Field Marshal personally and to enquire about his health and do the basic monitoring of his vitals and know about his fresh complaints/ symptoms. Those days, he's given a separate room for his quasi permanent stay in the command hospital Bangalore during his last days. I used to wait eagerly to meet him in the evening... The time scheduled for his visit used to be 8pm. Whenever I met him, I found  him very cheerful and extremely friendly and he had always offered me  a drink as a mark of courtesy which I always refused, firstly, being a teetotaller and secondly being on duty. His only complaint used to be "  Doctor.... Phain.... ( Pain ... He used to speak British English) here and I've phain here ... Do something. And yes, he used to talk about his paltan days very fondly and used to tell me never to accept a second prize in any competition... Because, in a war, a soldier never gets a second prize and yes, in war, there's no second prizes ever. I still follow his advice till date and I've never accepted a second prize in any competition till now ๐Ÿ™‚๐Ÿ™๐ŸผCol Pradeep Pendyala, Professor of Anesthesiology!!

Military Hospital Agra Days

 Lieutenant Akshit ( name changed to protect identify please ๐Ÿ™‚) I was posted at Military Hospital Agra in 1995 and was staying at officers' Mess along with my family before allotment of a permanent accommodation). I was working there as graded specialist in Anesthesiology. Life was going on as usual. Lieutenant Akshit was posted there along with three more interns from Armed Forces Medical College for their one year internship. I was a bit close to all of them. One of them was Lieutenant Pipariya ... He's excellent in Surgery... I always encouraged him to do more surgery under supervision of the Senior Surgeon Wg Cdr Ray. I must have given myself anaesthesia to many cases operated upon by lieutenant Pipariya under supervision of senior surgeon of course. Much later, after many years, I came to know that Lieutenant Pipariya opted for some other branch other than surgery... I was hugely disappointed. Well that's life. To make my Blog a bit interesting let me share with you one more incidence of the same period. As I have mentioned earlier about Lieutenant Akshit, it's related to him. Even in those days, I was very good at Mimicry. I called lieutenant Akshit on landline once posing as a reporter of Dainik Bhaskar who wants to interview him on his fabulous work at military hospital Agra. He's convinced that I was actually a reporter. Next day, we were all getting ready in the morning to go to the hospital for work but Lieutenant Akshit was well dressed and waiting eagerly to meet the Dainik Bhaskar reporter ๐Ÿ™‚Col Pradeep Pendyaja...  I'm a proud member of an elite group... ISA Cultural Club... It's a brainchild of Dr Naveen Malhotra sir. It's approximately hundred like-minded members. All are very talented and were handpicked by the admin. We enjoy sharing our creative work everyday. I usually make two new videos everyday and share them in this group. Besides this, the thing that keeps me going is teaching. I love teaching Anaesthesiology. This year, we'll have two more batches of NEET qualified post graduate students. True Leaders

 Have you ever seen a MBA degree holder being a very successful corporate leader. Have you ever seen a topper of the university being very successful in real life. Bill Gates, Steve Jobs and Dhirubhai never had big degrees but look at the heights they could reach. Leadership can't be taught, explained or instilled... It comes naturally. With the rise in cutthroat competition in almost every field, we need natural leaders, skilled people, not MBA graduates or University toppers. We need people with excellent communication skills, inspiration excellence and people who can influence the society. Ayyo Paapam

 I've done my MBBS from Andhra Medical College ( 1975-80 Batch ). After MBBS. Joined Indian Army Medical Corps in 1982. During my first annual leave, I visited my alma mater... Andhra Medical College. Met many of my friends and my classmates and a few professors. Moreover, now,  I was a Captain ... not just a Doctor. Met our professor of Medicine, Dr Mastan Rao. He asked me about my career and when I told him that I have joined Army ... His first reaction... " Ayyo Paapam". Which means, ohhh, I'm sorry for you ! Those days, anyone joining army meant only one thing... that he's been a failure everywhere. I had one more issue after joining Army.... All the girls which were offered to me when I was pursuing MBBS were no more available, because, in Andhra Pradesh nobody wants their daughters to be married to an Army Guy. I lived as a forced bachelor for three years with no suitable matches ( rather none ). Well, my first pelli choopulu ( seeing a bride for matrimonial alliance) was held in 1985. The girl was from Nagpur although originally from Vizag ... They had no issues with me being an Army officer. I saw the girl and immediately approved her. When we reached home, my parents told me that I can still see some more suitable girls if I wish. I told them, if I had said once yes, it means only YES. And I got married๐Ÿ™‚๐Ÿ™‚

PS. I'm the same person even after 37 years of married life but Andhra Pradesh has definitely changed a lot... Now, they're giving their daughters to Army guys for marriage๐Ÿ™‚๐Ÿ™๐ŸผCol Pradeep Pendyala

English bloopers

 Being in Army for 33 years, I've worked in every corner of India. Hindi and English are common languages that unite India. Interestingly, English is spoken differently in each state of India. Some are extremely confused with its pronunciation. Most of the  Andhraites pronounce Against as Agnaist. National is always pronounced as Neitional and Nation is pronounced as Naytion. This reminds me of our Associate Professor, Dr Venkat Rao garu of Andhra Medical College of Physiology department used to say ... Asked not, asked not, you asked this question... Professor is often spelt as Proffesor. When it comes to English, nobody can beat the Bloopers of Bengali English. There's a joke circulating in WhatsApp that recent accidental launch of a missile from India to Pakistan was the strange pronunciation of lunch by a Bengali commanding officer ...  All of us live two lives... First one everyone knows... Second one, we start living only when we suddenly realise that we've only one life and it's about to be over. Trust me, our second life is very enjoyable and very fruitful. We suddenly become very magnanimous and forgiving. We suddenly realise the importance of family and friends in our life. We develop deep understanding of the world around us and we also start appreciating the nature, birds and animals around us. We start respecting food and love eating fruits. Virtuous Cycle

 Twelve years ago, when I joined Facebook, my wife didn't approve of it ... My own son didn't like it. I was about to retire from my army life and I was not sure what I would be doing after my retirement. But after joining Facebook, I met people like Ampat Koshy  sir and Archna Pant ma'am who were exemplary in their English and Hindi respectively. Meeting them online was the turning point in my life. I developed a passion for English as well as for Hindi. I started reading serious literature which I had never done earlier. My English and Hindi considerably improved. And a virtuous cycle began in my life. There were chain of events in my life which I never dreamt of earlier... One desirable event leads to another... He knows a lot about specialties as paediatrics, obstetrics, cardiac or critical care ๐Ÿ’Š๐Ÿ’‰

Anaesthesiologists work in operating rooms, providing pain relief during surgeries.

They also work in intensive care units and in labor and delivery units  as well as providing care for patients suffering from chronic pain. Anaesthesiologists collaborate with other doctors and surgeons to coordinate treatment and care for patients. They must be familiar with all types of surgeries, Emergency cases and organ transplants. Covid

 COVID has come as a Tsunami in everyone's life...There won't be a single person who's not affected by it in this planet ... Many lost their dear and near ones and many lost their livelihood. In my case, I was devastated in altogether different ways... my regular tennis ๐ŸŽพ stopped for good... my regular visits to our college library stopped and my favourite restaurant, Mama's kitchen next to my residence was closed forcing me to start a Zomato. Our classes became online and regular interactions with my friends and relatives has come to a naught. My shuttling between Eluru and Secunderabad has decreased considerably. I lost few of my dear classmates and relatives which made me very very sad. Vaccination after vaccination but no guarantee against it... Like someone said, we lost a doctor recently, he was fully vaccinated, had he have not taken the vaccination, it could have been worse ๐Ÿ˜€๐Ÿ˜€
