Sir, pulse mein maza nahee aa raha...which means I am not liking the pulse. If Warrant Officer Chanda our senior most Operation Room Assistant had said this...it used to be most ominous for the prognosis of the patient those days. I am talking about 1989 when we had only monitor in the operation theatre available...Hand On Pulse. We never had those days pulse oximeter, NIBP, Capnography, Cardiac monitors and other multiple monitors. We used to rely on our clinical acumen and constant manual measurement of blood pressure and pulse. Interestingly, we had less number of anaesthetic mishaps those days. These days, more importance is being given on various investigations before a correct diagnosis or a provisional diagnosis. Today, we need MRI report, CT Scan report, Echocardiography report and countless other investigation reports before we diagnose a patient...our clinical acumen has taken a backseat. Those days, none of these investigations were available, we depended wholly on our clinical examination and few relevant investigation but with better outcome...Col Pendyala Pradeep, Anaesthesiologist.
